I can't speak for medieval table service but nowadays the bread bowl or taco bowl comes on a plate. It's just an affectation.

I am reminded of the time I went out to lunch with a friend and she ordered the taco bowl because it was "salad" and she was on a diet. Nevermind the deep fried shell, the heaps of sour cream and all the beans and cheese. Nowadays I can't see one without thinking of Trump in the early days of his campaign trying to appeal to hispanics by showing off the Taco Salad in the Trump Tower restaurant.
