I've actually toyed with the idea of making tags, with people's names printed on them, that would fit around the stem. Covered with either plastic document laminate or even clear packaging tape (to protect from drips) and a split hole that would fit around the stem (like the bread bag closures). Graphics could be put on them and people could keep them and reuse them. Duplicates could be made if multiple stems are being used. Almost sounds like a Martha Stewart thing.
My last party we pulled out a sheet of alphabet stickers. I put everyone's initial on their glass. Actually most of them are still on the glasses, so I've been able to get extra use out of them.

A number of years ago a relative made each of the families sets of charms that related to their interests. I use them when there are more than four of us in a mingle situation. I have not used them enough to notice any scratching on the stem, though I suspected that might be a risk. For Christmas we received an new set, because they resemble beach glass of which I am fond. Instead of a metal loop they are a helix shape that wraps around the stem presumably they will not harm the glass. We haven't used them yet, but they are attractive and look festive.

Plastic collars just as you describe do exist. They wouldn't be as fancy as custom made but just mark on them with wet-erase pens and rinse off for next time.
